What is this resource?

Covenant Community Care is a medical center offering a comprehensive range of integrated medical, dental, and behavioral health services. As a faith-based, non-profit Federally Qualified Health Center, their mission centers on providing affordable and quality care to all individuals, irrespective of their ability to pay or insurance status. Patients can access primary medical care, including preventative services, chronic disease management, and women’s health, alongside dental services. They also provide counseling and substance use treatment.

Resource Pricing

Covenant Community Care accepts most insurance plans. They also offer a sliding fee program- a program offered to individuals who do not have insurance. Your level of sliding fee is determined by income and household size. Your sliding fee will vary depending on where you fall on the scale.
